King’s Fund provides new framework for improving health and social care systems for older people

The King’s Fund says, “Our fragmented health and care system is not meeting the needs of older people, who are most likely to suffer problems with co-ordination of care and delays in transitions between services.  Whole-system changes are needed to deliver the right care at the right time, and in the right place, to meet older people’s care preferences and goals.”

The report, “Making our health and care systems fit for an ageing population,” sets out goals for nine areas of care, specifically for local service leaders, gives examples of creative solutions that work, and discusses major barriers to making integrated care a reality.
